To enter and be eligible for prizes: Fill out the entry form inside your DISCOVER INDY™ Savings Book, or pick up an official entry form at Discover Indy - Sweepstakes, 9760 Mayflower Park Drive, Carmel, IN 46032 . No purchase necessary. Winning flights from INDIANA only. Limit one entry per person. Your entry form must be submitted by November 15, 2011. Only official entry forms will be accepted. Facsimile and mechanically reproduced entries are prohibited and invalid as are multiple and illegal entries. Automated entries are not valid entries. Winners will be selected in a random drawing in January of 2012. You must be 18 years or older to enter. Prizes must be redeemed by December 31, 2012.
